"We turned to CARE when we received a large grant that required multiple forms of support, from evaluation to vetting technology vendors to facilitating meetings with the tech group. They also created a rubric tool to guide our eRegistry Technology AdHoc experts in evaluating advanced care planning technology platforms for the launch of our eRegistry that creates rapid access for POST (Physician Orders for Scope of Treatment) and Advanced Directives. The craft and impact of primary prevention efforts and systems change is often a difficult story to tell.
