School of Medicine Greenville

Registrar

The School of Medicine Greenville Registrar's Office adheres to the University of South Carolina Registrar's Office guidelines. We maintain stewardship and integrity of student academic records and manage effective policies and practices regarding registration, academic progress, and graduation.

Fellowship applications require the submission of your transcript and Medical School Performance Evaluation (Dean’s Letter).

You may request these documents through your online Medical Institution Document Upload Service (MIDUS) account. When submitting the request via MIDUS, use registrar@greenvillemed.sc.edu as the contact for both the MSPE and transcript.

Diplomas are issued based upon the legal name in the Registrar's Office. You will receive your diploma at graduation, with several copies. If you need a duplicate diploma, please contact Main Campus Registrar.

Current students may request a Letter of Good Standing/Enrollment Verification by completing a request using the Letter of Good Standing or Enrollment Verification form.

The Registrar's Office is responsible for maintaining a student's or former student's legal name in official school records. Academic records that contain the legal name of the student or former student include transcripts, evaluations and diplomas. Currently enrolled students and graduates can request that their official school records be updated to reflect a new name provided they submit proof of their legal name change. In order to request a name change on your official school records, you must complete the Name Change Request form and submit to us. Please do not send the form to the main campus as indicated on the form.

Protecting your privacy has never been more critical. While we work diligently to prevent data disclosures, you’re urged to be equally vigilant in your actions both on and off campus.  The School of Medicine Greenville adheres to the University of South Carolina FERPA policy.

The Registrar's Office handles the registration of all students into USC's SRS system Banner. M1 and M2 students are added to courses in both Banner and OASIS by the Registrar's Office.

M3 and M4 students work with the Office of Medical Education to enroll in courses in OASIS. The Registrar's Office handles the registration of these courses in Banner.

The Registrar processes required paperwork for state licensure and/or medical education verification.  All forms should be sent to the Registrar email.

Students are encouraged to regularly review their academic transcript via their Self Service account. The main campus in Columbia processes all official transcript requests. Please visit the University Registrar for more information.
