Important Student Forms
Intent to Return: This form notifies University Housing if you intend to return to your position next academic year.
Payroll Inquiry: This form should be completed if you have questions or concerns regarding your pay. Please allow 24-48 hours for a response.
Hire / Departure Form: Completed by supervisor for new or depaturing employees.
Dual Employment Request Form: Students complete form if working more than one job on-campus.
Exit Interview: Departing students fill out if they would like to share their housing experience with HR.
Position Description: Used to create new student positions and seek approval from HR and leadership.
Student Employee Performance Review: Form to provide meaningful feedback to your student employees, regarding their student performance in their job.
