Dear UofSC Columbia Faculty and Staff,
In the coming months, Student Health Services will transition to University Health Services to better serve the entire university community. One of our goals with this change is to expand health and wellness services available to all students, faculty and staff on the Columbia campus. As we prepare for this expansion, we are seeking your feedback regarding the programs and services that you would utilize. Data collected from this assessment will be used to develop health and wellness programs and services to help faculty and staff achieve and maintain a healthy lifestyle.
The Faculty and Staff Wellness Culture and Interest survey is confidential and should only take about 20 minutes to complete. You have until April 18, 2021 to complete this survey. Your participation in this survey is voluntary. To access the Faculty and Staff Wellness Culture and Interest survey use the following link:

To express our appreciation for your participation, you will have the opportunity to enter a drawing to win giveaways including Target, Starbucks and Amazon gift cards.
It is our sincere hope that faculty and staff at the university will participate in this health assessment as the information gathered has the potential to greatly benefit the health and well-being of the Carolina community. If you have further questions about this survey or how the data will be used on UofSC’s campus, please contact Gamecocks Live Well at livewell@mailbox.sc.edu.
Thank you for your cooperation.
Sincerely,
Deborah Beck, MPA, Ed.D, FACHA
