There are several important documents you will need in managing payroll responsibilities for your department or for making changes to eligible deductions for yourself. We have made some key forms easily accessible.

HCM Access Form [pdf]

The TL/ABS Department Approver will have the ability to create and approve employee timesheets within their department in situations where the employee’s supervisor is unavailable. To serve in the role as the TL/ABS Department Approver an HCM HR/Payroll PeopleSoft Access Form, which includes a Statement of User responsibility, must be completed, and submitted. Each department can assign up to three individuals to serve as the TL/ABS Department Approver.

Access to departmental payroll data through the General Accounting Intranet.

Statement of User Responsibility [pdf]

Acknowledge that the university's policy prohibits the disclosure of sensitive information.

W-4 Withholding Form [pdf]

This pdf version of the form is only available for international employees. All other employees, please use Employee Self Service in HCM PeopleSoft.

SC W-4 Withholding Form [pdf]

Required to determine the  state tax withholdings deducted from your earnings.
