Nutrition Supports for Health Clinics Toolkit: 2nd Edition

This is a 2nd edition toolkit of healthy eating policy, systems, and environmental change strategies encouraged by the national SNAP-Ed program for obesity prevention and addressing food insecurity. It includes 1) an overview of SNAP, SNAP-Ed, and policy, systems, and environmental change strategies; 2) guidance on assessing readiness and ensuring sustainability of implemented strategies; 3) a description of 12 nutrition support strategies conducive to the health clinic setting; 4) evaluation resources; and 5) case studies of health clinics that have implemented nutrition and physical activity supports.
