Nutrition Supports for Health Clinics Toolkit

This is a toolkit of healthy eating policy, systems, and environmental change strategies encouraged by the national SNAP-Ed program for obesity prevention and addressing food insecurity. It includes 1) an overview of SNAP, SNAP-Ed, and policy, systems, and environmental change strategies; 2) a description of 12 nutrition support strategies conducive to the health clinic setting; and 3) case studies of health clinics that have implemented nutrition and physical activity supports.

Thank you for your interest in this toolkit! If you have any questions or comments, please direct them to Carrie Draper, Principal Investigator of the University of South Carolina's SNAP-Ed Implementing Agency, at draper@mailbox.sc.edu or 803-777-2413.

In an effort to track dissemination of the toolkit, please complete the below questions and then you will be able to download the toolkit. Also, when sharing with others, please send them the link to this page and not the document itself.
