The USC Brain Health Network is here to work with you and your family doctor, or primary care provider, to give you access to cognitive screening and evaluation services to help meet your unique needs.
Getting a timely and accurate diagnosis is important when it comes to caring for memory issues and concerns. The first step you need to take is to see your primary care provider.
If your provider refers you to the USC Brain Health Network, we'll assess your cognitive concerns and perform a thorough screening and evaluation.
After your screening and evaluation, we will develop a recommended care plan to meet your and your caregiver’s or family’s needs and goals.
We schedule three USC Brain Health Network appointments with the patient and/or the caregiver to determine care plan recommendations. These results are shared with your referring provider to manage your care or provide next steps for you.
First appointment
Your first appointment will be a phone call with one of our Speech-Language Pathologists. They’ll speak with you about any memory changes you’ve had or concerns you may have. This appointment usually takes about 45 minutes.
Second appointment
Your second appointment will be in-person with a Speech-Language Pathologist. This appointment typically takes about 90 minutes.
Third appointment
Your third appointment will be in-person with a health care provider. They will have evaluated your screening results and be prepared to discuss them with you.
Our Certified Community Health Worker will also be at this appointment. They will be there to provide the patient and/or their caregiver support by recommending community resources. This appointment usually takes about one hour.
When your three appointments are complete, our team will send your screening results and recommendations to your provider.
