Biography

Takia Woods is a Certified Community Health Worker (CCHW)  with USC Brain Health. As a CCHW, she works directly with the Brain Health nurse practitioner in personalizing care plans for each patient and caregiver. Her motto is "meeting the patient and caregiver where they are, and allowing them to be able to make decisions when it comes to their health." She also ensures a warm hand-off is made for patients from USC Brain Health to community-based resources and organizations. Additionally, she provides external support to assist with patients Activities of Daily Living. 

Woods is a graduate of the University of South Carolina with a Bachelor of Art in Public Health and Minor in Psychology with Leadership Distinction in Professional and Civic Engagement. She completed her undergraduate internship at Southern Lakes Therapy, where she worked with children with disorders, and assist adults with physical therapy and occupational therapy activities. Woods recently received certification as a community health worker through the Center for Community Health Alignment. She is currently attending Capella University working toward a Master of Science in Public Health and completing her 80-hour practicum with the Department of Mental Health at the G. Werber Bryan Forensic Hospital. 

Woods has been employed by the Department of Social Services as an Eligibility Specialist and SNAP Employment & Training Coordinator. She was in charge of Anderson, Oconee and Pickens counties where she worked directly with clients and provided assistance in obtaining education for High School Diplomas, GEDs and certifications.
