Appointments

You can schedule most appointments online through your account on MyHealthSpace or by phone. Some appointments may require a referral from your primary care physician.

Schedule an Appointment Online

Some appointments must be made over the phone, including those for counseling, psychiatry, faculty/staff wellness and nutrition.

Your Appointment

Bring your CarolinaCard to check in and a copy of your insurance card with you.

Before You Arrive

Fill out the forms on MyHealthSpace, including the Health History, Treatment Agreement & Notice of Privacy Practices and Permission to File Insurance.

Running Late

If you are more than five minutes late, you may need to reschedule your appointment.

Missing an Appointment

Let us know as soon as possible if you can't make your appointment. There are fees for missing certain kinds of appointments without letting us know.

Doctor's Notes

We cannot excuse you from class or other academic requirements. On request, we can give you a validation slip with the date and time you came in for an appointment plus any limitations your medical care may cause.

 

Who Can Make an Appointment

Any Columbia campus student enrolled in at least one credit hour in a degree program can make an appointment.
