Your application for meal plan modifications or accommodations should include documentation from a qualified medical professional, on letterhead, signed and dated, including both a diagnosis and a list of what the medical professional does not want you to consume. The list can include specific items such as gluten or peanuts, a group of foods such as dairy, or food preparations such as fried or processed.
Once your application has been reviewed, you will be notified by phone or email about your status.
