• SIPA Behavior Form (required)
  A copy of this form must be signed by each student, his/her parent and adviser. Behavior forms should be turned into SIPA personnel at the convention desk. Teachers/advisers should make a copy of each signed behavior contract to keep in your files.
  
  
• Hotel Reservation Form (required)
  To reserve hotel rooms, fill out this form and follow the instructions to send it to the hotel office.
  
  
• Individual Student Information Sheet (required)
  Please make copies of this form for each student and bring them with you to the convention. The College of Mass Communications and Information These forms should be turned into SIPA personnel at the convention desk. Studies wants to be able to correspond with students who attend the convention.
  
  
• Student Officer Nomination Form
  All candidates for office must currently be juniors. Criteria are explained on the form.
